All ICA participants, whether coming independently, with a group or through a college / university, must provide the following completed forms and information to firstname.lastname@example.org at least 2 weeks prior to attending their program.
Emergency Contact & Medical Information
This form provides us with emergency contact information and any medical conditions that could affect your stay at ICA. This is particularly important if you have allergies that could be triggered by certain foods or events like a bee sting, or if you have chronic conditions such as a heart condition or diabetes in which case having advance knowledge of your condition could facilitate helping you in the event of an emergency.
All participants must sign a liability waiver releasing the International Center for the Arts from liability while participating in ICA programs and trips.
Proof of Insurance / Travel Insurance
We require that all our participants provide proof of health and travel insurance, along with policy information upon registration in case of emergency. One or more of your policies must include coverage for health care, emergency evacuation, theft, baggage loss, trip cancellation / interruption.
Itinerary and Passport
In addition to the forms above, all attendees must email us the following information:
Your arrival and departure times to FCO International
Photo/Scan of passport that clearly shows photo and identification information
Indicate if you are not arriving to Italy via FCO international terminal, so that we can provide directions via bus.
